In consideration for being permitted by Eminent Training + Athlete Development and Eminent 7 on 7 (hereinafter referred to as "the Company") to participate in any activities, events, or programs offered by the Company, I agree to the following terms and conditions: 1. Assumption of Risk: I acknowledge and understand that participation in athletic activities involves inherent risks, including but not limited to physical injury, illness, or death. I voluntarily assume all risks associated with my participation in activities organized by the Company. 2. Medical Clearance: I certify that I am physically and medically fit to participate in the activities offered by the Company. I understand that it is my responsibility to consult with a physician prior to participating in any physical activity if I have any concerns about my health or physical condition. 3. Release of Liability: I hereby release, discharge, and hold harmless the Company, its owners, officers, directors, employees, agents, and representatives from any and all claims, liabilities, damages, losses, or expenses arising out of or in connection with my participation in Company activities, including but not limited to personal injury or property damage, whether caused by negligence or otherwise. 4. Indemnification: I agree to indemnify and hold harmless the Company from any and all claims, liabilities, damages, losses, or expenses, including attorney's fees, arising out of or in connection with my participation in Company activities or my breach of any terms of this waiver. 5. Photo and Video Release: I grant the Company permission to use my likeness in photographs, videos, or other media for promotional purposes related to the Company's activities, without compensation or notification. 6. Governing Law: This waiver shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia, without regard to its conflict of law principles. 7. Severability: If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
